Ovarian mucinous tumor with sarcomatous mural nodules (patient 2). The adenofibromatous areas contain atypical intestinal-type mucinous glands with stratification in keeping with a borderline tumor [a]. The invasive lesions are characterized by dyscohesive atypical cells [b] and atypical mucinous glands admixed with the myxoid components [c]. The mural nodules are composed of uniform short spindle cells separated by abundant myxoid matrix [d]. The hyperchromatic tumor cells harbor a high nuclear-cytoplasmic ratio [e]. Focal spindle cells with esinophilic cytoplasm arranged in fascicles were suggestive of smooth muscle differentiation [f]. The neoplastic cells in the mural nodules are CD10+ [g] and have a high Ki-67 index [H]. (H&E: a × 5, b, c & e × 200, d & f × 100; immunohistochemistry, g & h × 200)
